Job summary

Elliot Health System is seeking a Physical Therapist for its outpatient Rivers Edge Rehab in Manchester, NH. The role focuses on evaluation, planning and implementing PT programs in orthopedics with options for mentorship and professional growth.

We offer a structured clinical ladder, dedicated documentation time, and a supportive team. Full-time schedule includes a 40-hour week with two evenings until 7 PM and a competitive benefits package.

Qualifications

• Graduate of an accredited Physical Therapy Program with Entry Level Degree.
• Licensed as a Physical Therapist by the state of New Hampshire.

Responsibilities

• Prepares for patient evaluation by conducting medical record review and identifying focus areas.
• Evaluates therapy needs and develops a written plan of care with short- and long-term goals.
• Updates treatment plan and re-evaluates patient progress with the care team.
• Establishes discharge plan and instructs patient and family in home exercise programs for optimal function.
• Provides supervision of therapy staff involved in the plan of care.
