Responsibilities

• Prepare for patient evaluation by reviewing records and identifying focuses and precautions.
• Evaluate therapy needs and develop a prioritized plan of care with goals.
• Update treatment plans and communicate with referrers and clinicians.
• Establish discharge plan and train patient and family in home exercise programs.
• Supervise therapy assistants and support staff in implementing care plans.
