Advocate Health in Illinois seeks a licensed Physical Therapist for a full-time outpatient role. You will evaluate patients, develop and implement individualized treatment plans, and coordinate with physicians and the care team to achieve functional goals.

Responsibilities include documentation, discharge planning, and participation in quality initiatives. A Doctorate in Physical Therapy and current state licensure are required; BLS is also mandatory.

Formación

• Doctorate degree in Physical Therapy from an accredited school; maintains CE requirements as mandated by state standards.
• Licensed Physical Therapist in the state; BLS for Healthcare Providers (AHA).
• Intermediate computer skills; strong communication, interpersonal, and organization abilities.

Responsabilidades

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment; monitors outcomes and adjusts the plan; communicates with physicians and teammates.
• Plans for discharge including home programs, teaching, and referrals; coordinates with stakeholders for optimal care.
• Documents per department policy; records patient care and progress; adheres to billing and productivity standards.
• Cleans up after treatment; operates modalities and equipment; ensures safety and infection control.
• Participates in performance improvement; manages caseload and coverage; supports professional growth and staff orientation.
• May supervise PT assistants and rehab aides; may supervise rehab students.
