Job summary

Qualifications

• Doctorate degree in Physical Therapy is required.
• Excellent communication, interpersonal and organizational skills.
• Demonstrated organization, decision-making and problem-solving abilities.
• Intermediate computer skills and familiarity with documentation systems.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary and/or integumentary evaluations.
• Establishes and implements a treatment plan based on evaluation results and functional goals; monitors outcomes and adjusts as needed.
• Completes discharge planning with home programs, patient/family teaching and referrals to community services.
• Maintains documentation according to policy and accreditation standards; records patient care and progress; handles billing and scheduling per site guidelines.
• Contributes to safety initiatives and adheres to infection control measures; may supervise assistants and rehab aides as needed.
