Job summary

Fairview Range in Hibbing, MN is seeking a Physical Therapist to join Outpatient Rehabilitation Services. You will provide patient-centered therapy to restore function and improve quality of life, working closely with a collaborative care team.

New graduates are welcome; the role offers professional growth within a supportive environment and a schedule of 80 hours over two weeks with 8-hour day shifts and rotating weekends. This is a full-time, on-site position.

Qualifications

• Physical Therapist licensure in the state of Minnesota or temporary MN license
• 0 to 1 year experience in Physical Therapy (new grads welcome!)
• Basic Life Support (American Heart Assoc or Red Cross)

Responsibilities

• Partnering with the patient and family, while performing the Evaluation/Re-evaluation.
• Developing an individualized plan of care through collaboration with the patient and their families.
• Partnering with patients and their families to provide skilled intervention.
• Implementing and modifying the plan of care, based on reassessment and patient response.
• Completing documentation in a timely manner per department standards.
• Coordinating care and communicating optimally with interdisciplinary team, for all aspects of patient care.
