Job summary

Fairview Northland Medical Center in Princeton, MN is seeking a casual physical therapist to support both outpatient and inpatient needs within our rehabilitation teams. The role involves examination, planning, intervention, and achieving patient-centered outcomes in collaboration with patients and families.

You will participate in a flexible weekend rotation with opportunities to pick up weekday shifts for outpatient and inpatient coverage.

Qualifications

• Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota
• Basic Life Support (American Heart Association or Red Cross)

Responsibilities

• Partners with patient and family during evaluation and re-evaluation
• Gathers pertinent data
• Performs examination
• Determines treatment diagnosis
• Develops an individualized plan of care with patient and family
• Analyses data to identify needs and risks
• Determines prognosis for recovery
• Sets individualized goals with patient and/or caregiver
• Individualizes plan of care considering developmental level and cultural differences
• Implements and modifies plan of care based on reassessment and patient response
• Coordinates care and communicates with interdisciplinary team
• Establishes discharge plan
• Delivers skilled intervention and implements plan of care
• Alters treatment as patient status changes
• Assesses learning needs and provides education
