Job summary

Fairview Health Services seeks an orthopedic physical therapist for outpatient clinics serving adolescents to adults across the Twin Cities metro area. MDT training is preferred; mentoring is available to develop MDT skills.

The role involves safe, evidence-based care within the scope of practice, including examinations, planning, interventions, and outcome measures. The position is 1.0 FTE with typical hours Monday–Friday, 8:00am–4:30pm, coverage across multiple locations.

Qualifications

  • Licensed Physical Therapist in Minnesota or temporary MN license.
  • BLS certification required (American Heart Association or Red Cross).
  • MDT training preferred; mentoring available for MDT exposure.

Responsibilities

  • Perform Evaluation/Re-evaluation with patient and family.
  • Gather relevant data and perform examinations.
  • Determine diagnosis and prognosis; develop an individualized plan of care.
  • Collaborate with interdisciplinary team and coordinate care.
  • Educate patient and family; modify plan as needed; establish discharge plan.

Job Overview

We are seeking a dynamic orthopedic physical therapist to join our outpatient flexible workforce team! The physical therapist will serve adolescent through adult patients across multiple outpatient, orthopedic therapy clinics in the greater Twin Cities metro area. Preferred candidate will have Mechanical Diagnosis and Therapy (MDT) training. Where MDT training and experience is limited, Fairview offers mentoring and development opportunities for professional growth. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

  • FTE: 1.0, (80 hours per pay period).
  • Schedule: Monday - Friday, generally 8:00am - 4:30pm; shift can vary +/- 30 minutes, depending on the needs of the clinic being covered.
  • Position provides coverage at various locations across the greater Twin Cities metro including, Blaine, Bloomington, Brooklyn Park, Burnsville, Eagan, Edina, Eden Prairie, Fridley, Golden Valley, Hugo, Maple Grove, Maplewood, Minneapolis, St. Anthony, St. Paul, and Woodbury.

Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Responsibilities
  • The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation
  • Gathers pertinent data
  • Performs examination
  • Determines treatment diagnosis
  • Develops an individualized plan of care through collaboration with the patient and family
  • Analyzes data to identify needs and potential risks
  • Determines prognosis for recovery
  • Establishes individualized goals with patient and/or caregiver
  • Individualizes plan of care considering developmental level and cultural differences
  • Implements and modifies plan of care based on reassessment and patient response
  • Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
  • Establishes appropriate discharge plan
  • Partners with patients and families to provide skilled intervention
  • Implements plan of care
  • Alters treatment/plan of care to reflect change in patient status and response to treatment
  • Assesses patient and/or caregiver learning needs and provides appropriate education
Required Qualifications
  • Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota
  • Basic Life Support (American Heart Association or Red Cross)
Preferred Qualifications
  • Doctorate of Physical Therapy
  • 1 year experience
  • MDT experience and training
