Job summary

Advocate Health in Charlotte, NC is seeking a full-time Physical Therapist to support outpatient rehabilitation services. The role requires a Bachelor's degree in Physical Therapy, an active state license, and BLS certification. Schedule is Monday–Friday, 8am–5pm, with a 40-hour work week.

Responsibilities include patient evaluations, treatment planning, documentation, and coordination with physicians and rehab staff. Supervision of PTA/ Rehab Tech staff is expected as needed.

Qualifications

• Clinical assessment and treatment planning based on medical records and patient needs.
• Accurate documentation of evaluations, progress notes and discharge plans.
• Coordination with physicians and rehab staff for patient care.
• Supervise PTAs and Rehab Tech as required by licensure.

Responsibilities

• Evaluate patients and establish care plans with appropriate modalities and goals.
• Gather data from medical records, physicians, patients and families.
• Provide evidence-based interventions and adjust plans as needed.
• Document evaluations, progress and outcomes; manage scheduling and discharge planning.
• Communicate progress to physicians and coordinate care with the team.
