Qualifikationen

• Bachelor's degree in Physical Therapy and valid state license required.
• BLS certification required.
• Certified Lymphedema therapist and outpatient rehabilitation experience preferred.

Aufgaben

• Evaluates patients and synthesizes information to establish a patient care plan with modalities, procedures, exercise programs, therapeutic groups, and education as warranted by the diagnosis and discharge needs.
• Gathers and interprets data from medical records, physician reports, patients and families as evidenced by documentation.
• Sets functional goals with the patient and provides evidence-based intervention with ongoing assessment and plan adjustments.
• Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient education, outcomes, discharge notes, and charges as needed.
• Communicates evaluations and progress to physicians, referral sources, and other clinical staff; coordinates schedules and discharge planning.
• Provides supervision of PT Aides and Rehab Tech/Aide as required for licensure.
