Job summary

Advocate Health is seeking a qualified physical therapist for the Outpatient Rehabilitation department at Lexington Medical Center. This outpatient position focuses on lymphedema care, with a Monday–Friday schedule and a 40-hour workweek.

Relocation assistance is available for qualified candidates. The role requires a PT degree from an accredited program, active licensure in the applicable state, and CPR/BLS Level C certification.

Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the applicable state
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Adheres to hospital standards and promotes teamwork.
• Provides patient care assessment and treatment with realistic goals.
• Documents evaluations, progress notes, and discharge notes.
• Maintains productivity and meets department budget and schedules.
• Participates in process improvement and shares best practices.
• Provides education and represents department on committees.
