Job summary

Patriot Outpatient Therapy is seeking an Outpatient Physical Therapist to provide in-home hospice evaluations and treatments across Mahoning County, Ohio. This PRN role emphasizes comprehensive assessments, interdisciplinary collaboration, and family education within a supportive, veteran-owned team.

The position offers flexible scheduling, mileage reimbursement, and a 401(k) with company matching. Candidates must be licensed, CPR certified, and able to travel to patient homes.

Qualifications

• Must be a graduate of a Physical Therapy program approved by the AMA Council of Medical Education in collaboration with the American Physical Therapy Association.
• Must be licensed or registered by the State.
• Must have criminal background check.
• Must have a current CPR (BLS) certification.

Responsibilities

• Provide patient services to community dwelling hospice patients in their homes for evaluations and treatment visits.
• Provide initial and ongoing comprehensive assessments of the patient's needs.
• Establish a treatment plan in consultation with the physician and revise it with the physician's approval as necessary.
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy.
• Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
• Teach, supervise, and counsel the family and patient in the physical therapy program.
• Write clinical/progress notes on the date of the visit and incorporate the same in the patient's record.

Education

Graduate of a Physical Therapy school approved by the AMA Council of Medical Education and the APTA

Job Conditions:

• The ability to drive
• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
• On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds

Qualifications:

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association
