Job summary

Patriot Homecare seeks a PRN Physical Therapist to provide home health PT services across Mahoning, Trumbull, and Columbiana counties.

You will perform assessments, develop therapy plans with physicians, educate families, and document progress within 72 hours. A PT degree, Ohio license, and CPR certification are required.

Qualifications

• Must be a graduate of an accredited Physical Therapy program.
• Must hold an active Ohio state license.
• Current CPR (BLS) certification required.
• Criminal background check required.
• Prior home health experience preferred.

Responsibilities

• Provide initial and ongoing assessments of patient needs including PT-specific measures.
• Develop and adjust treatment plans in collaboration with physicians.
• Contribute to interdisciplinary care planning for physical therapy.
• Perform diagnostic and prognostic tests relevant to PT.
• Educate and supervise patients and families in the therapy program.
• Document visit notes and integrate into the patient chart within 72 hours.

The responsibilities of our Physical Therapists include:

• Provide initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments
• Establish a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy
• Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests
• Teach, supervise, and counsel the family and patient in the physical therapy program
• Write clinical/progress notes on the date of the visit and incorporates same in the patient's record within 72 hours.
  

Job Conditions:

• The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required
• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
• On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds

Qualifications:

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association
• Must be licensed or registered by the State
• Must have at least one year of institutional/home health experience
• Must have criminal background check
• Must have a current CPR (BLS) certification
