Patriot Outpatient Therapy in Ravenna, OH, is seeking an Outpatient Physical Therapist for hospice patients on a PRN basis. You will provide in-home evaluations and treatments, develop and adjust treatment plans with physician collaboration, and support an interdisciplinary team.

The role requires a PT degree, State licensure, CPR/BLS, and a background check. Mileage reimbursement and a 401(k) with 4% company matching are offered. This position emphasizes community impact and professional growth.

Qualifications

• Must be a graduate of a Physical Therapy school approved by the AMA in collaboration with the APTA.
• Must be licensed or registered by the State.
• Must have criminal background check.
• Must have a current CPR (BLS) certification.

Responsabilités

• Provide patient services to hospice patients in their homes for evaluations and treatment visits.
• Provide initial and ongoing comprehensive assessments of the patient's needs.
• Establish a treatment plan in consultation with the physician and revise it with physician approval as necessary.
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy.
• Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
• Teach, supervise, and counsel the family and patient in the physical therapy program.
• Write clinical/progress notes on the date of the visit and incorporate the same in the patient's record.

The responsibilities of our Physical Therapists include:

Job Conditions:

• The ability to drive
• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
• On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds

Qualifications:

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association
• Must be licensed or registered by the State
• Must have criminal background check
• Must have a current CPR (BLS) certification
