Job summary

Patriot Outpatient Therapy seeks a licensed Physical Therapist to provide home health services in Columbiana County. You will assess patient needs, develop and adjust treatment plans, and collaborate with physicians to achieve rehabilitation goals.

The role involves educating patients and families, documenting progress, and working in a community-focused team. PRN hours with mileage reimbursements and 401k benefits available.

Qualifications

• Must be a graduate of a Physical Therapy school approved by the AMA Council and APTA.
• Must be licensed or registered by the State.
• Must have criminal background check.
• Must have a current CPR (BLS) certification.

Responsibilities

• Provide services to patients in their homes, hospice, and non-hospice patients, including patients with multiple disabilities.
• Provide initial and ongoing comprehensive assessments of the patient's needs.
• Establish a treatment plan in consultation with the physician and revise it with physician approval.
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy.
• Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
• Teach, supervise, and counsel the family and patient in the physical therapy program.
• Write clinical/progress notes on the date of the visit and incorporate the same in the patient's record.

The responsibilities of our Physical Therapists include:

Job Conditions:

• The ability to drive
• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
• On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds

Qualifications:

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association
• Must be licensed or registered by the State
• Must have criminal background check
• Must have a current CPR (BLS) certification
