Job summary

Patriot Hospice is seeking a Registered Nurse Case Manager (PRN) to plan, coordinate, and provide hospice care in accordance with each patient\'s plan of care. You will work with interdisciplinary teams, patients, and families to ensure appropriate and timely services at home.

The role requires an active RN or LPN license, valid CPR/BLS certification, and familiarity with Medicare/Medicaid guidelines. You will participate in IDG meetings and on-call coverage as needed, supporting patients in

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Must be licensed in the State as a Registered Nurse or Licensed Practical Nurse.
• Knowledge of Medicare and Medicaid guidelines is required.
• Must have a criminal background check.
• Current CPR (BLS) certification is required.

Responsibilities

• Plan, coordinate, and provide hospice care in accordance with the patient\'s plan of care.
• Coordinate hospice care by working with all team members, patients, and family.
• Evaluate the effectiveness of hospice services for patient and family.
• Submit complete and accurate clinical notes in a timely manner.
• Participate in IDG and case conferences.
• Perform on-call duties (including holidays) per policy and agency need.

As a Registered Nurse Case Manager, you will plan, coordinate, and provide hospice care in accordance with the patient's plan of care.

As a PRN Nurse, you will:

• Coordinate hospice care by working with all team members as well as patients and family members.

• Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.

• Perform evaluations for hospice care along with informational visits as needed.

• Submit complete and accurate clinical notes in a timely manner.

• Participate in IDG and case conferences.

• Perform on-call duties (including holidays) per policy and agency need.

• All other duties as assigned.

Qualifications

• Must be a graduate from an accredited School of Nursing

• Must be licensed in the State as a Registered Nurse or Licensed Practical Nurse

• One or more years of experience in hospice preferred

• Must have knowledge of Medicare and Medicaid guidelines

• Must have a criminal background check

• Must have a current CPR (BLS) certification

Job Conditions

• Must have a driver's license and be willing and able to drive to patient's residences

• The ability to access patients' homes which may not be routinely wheelchair accessible is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care

• Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and turning of patients
