Lexington Health in Columbia, SC is seeking a licensed Physical Therapist to provide outpatient services on a PRN, day-shift basis. The role emphasizes restoration of function after trauma or disability, with a focus on patient education and teamwork within the care continuum.

The ideal candidate holds a BS in Physical Therapy, SC licensure, CPR certification, and is prepared to work across adult and geriatric populations in a comprehensive care network that includes hospitals and clinics.

Qualifications

• Minimum Education: Bachelor of Science Degree or Higher in Physical Therapy.
• Graduate of a PT program approved by the American Physical Therapy Association or equivalent per SC board.
• Licensed in South Carolina by the SC State Board of Physical Therapy Examiners; CPR Certification.

Responsabilités

• Provides direct physical therapy to patients upon referral by a physician or advanced practice provider.
• Evaluates patients using physical therapy tests and measurements (e.g., muscle testing, range of motion, functional ability).
• Develops physical therapy treatment plans and administers treatments to relieve pain and restore function.
• Documents patient treatment data and discharges summaries; educates patients and families for home programs.
• Supervises and monitors patient care when carried out by a physical therapist assistant; collaborates with interdisciplinary team.
• Takes orders from physicians' offices and hospital care teams; schedules patients; represents department in meetings.

Formation

Bachelor of Science Degree or Higher in Physical Therapy

Graduate of a PT program approved by the American Physical Therapy Association or other educational program which the South Carolina board considers to be equivalent

Licensed in the State of South Carolina by the SC State Board of Physical Therapy Examiners
