Job summary

Patriot Homecare seeks a dedicated Homecare Physical Therapist to assess and treat patients in Trumbull County. The role requires OASIS experience, a PT degree, and Ohio licensure. You will collaborate with physicians and an interdisciplinary team to create and adjust treatment plans while guiding families in therapy.

Travel to patient homes is required. The position offers full-time hours, a competitive salary, mileage reimbursement, and comprehensive benefits.

Qualifications

• Must be a graduate of a Physical Therapy program approved by the AMA/APTA.
• Must be licensed or registered by the State of Ohio.
• Must have at least one year of institutional/home health experience.
• Must pass criminal background check.
• Must have current CPR (BLS) certification.

Responsibilities

• Provide initial and ongoing comprehensive assessments including OASIS.
• Establish a treatment plan in collaboration with the physician and revise as needed.
• Assist in development and implementation of the interdisciplinary patient care plan for Physical Therapy.
• Assist physician evaluating patients using diagnostic/muscle-nerve-joint and functional tests.
• Teach, supervise, and counsel patient and family in the therapy program.
• Write progress notes on the date of the visit and file within 72 hours.

The responsibilities of our Physical Therapists include:

• Provide initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments
• Establish a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy
• Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests
• Teach, supervise, and counsel the family and patient in the physical therapy program
• Write clinical/progress notes on the date of the visit and incorporates same in the patient's record within 72 hours.

Job Conditions:

• The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are required
• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care
• On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds

Qualifications:

• Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association
• Must be licensed or registered by the State
• Must have at least one year of institutional/home health experience
• Must have criminal background check
• Must have a current CPR (BLS) certification
