Job summary

Lexington Health in West Columbia, SC seeks a licensed Physical Therapist for a full-time day shift. You will deliver outpatient PT services, assess patients, and develop individualized treatment plans.

Work with a multidisciplinary team, document progress, educate patients and families, and supervise assistants as needed. A valid SC PT license and CPR are required; a sign-on bonus is offered.

Qualifications

• Bachelor of Science in Physical Therapy or higher.
• Licensed in South Carolina by the SC State Board of Physical Therapy Examiners.
• CPR certification.

Responsibilities

• Provides direct physical therapy to patients referred by a physician or APP.
• Evaluates patients using standard PT tests and measurements.
• Develops and implements PT treatment plans.
• Instructs patients and families in home programs.
• Supervises PT assistants and communicates with the interdisciplinary team.
• Schedules patients and participates in department meetings.
