Ready to join? We are seeking a full-time Outpatient Physical Therapist to join our rehabilitation team. This position will be based at the Park Ridge, IL clinic. Our PTs plan, implement, and evaluate patient care programs aimed at increasing, maintaining, or restoring function, minimizing or preventing disability, and helping patients reach their maximum performance levels.

Essential Duties and Responsibilities

1. Evaluate patients and gather appropriate information to determine accurate clinical diagnoses and treatment plans.
2. Establish appropriate, functional, and objective goals based on clinical examinations.
3. Devise detailed treatment and care plans for patients.
4. Provide exceptional patient care and service.
5. Maintain friendly, cordial relations with clients and employees, fostering a positive work environment through respectful communication.
6. Cultivate relationships with patients and the community.
7. Communicate continuously with referring physicians and other referral sources to ensure consistent care.
8. Maintain patient charts, clinical logs, and checklists for compliance.
9. Ensure patient confidentiality regarding medical records in accordance with company policies and HIPAA guidelines.
10. Communicate effectively in a professional setting.
11. Perform additional duties as required.

Education and/or Experience

1. Bachelor’s, Master’s, or Doctorate Degree from an accredited Physical Therapy Program.
2. Current Illinois Physical Therapy license or willingness to apply.
3. CPR certification.
