Job Description

The Physical Therapist embodies the mission, vision, and values of NM, adheres to the Code of Ethics and Compliance Program, and complies with all relevant policies, procedures, and standards.

  • A licensed physical therapist complying with the Code of Ethics/Standards of Practice as outlined by their professional organization and the State of Illinois.
  • Responsibilities include direct patient care, teaching, committee participation, and process improvement.
  • Serves as a role model for therapists and interns in professional, clinical, and leadership activities.
Responsibilities
  • Demonstrates NM values: teamwork, patients first, integrity, excellence.
  • Maintains a friendly, professional demeanor and appearance.
  • Provides accurate information, updates, and directions to patients and customers.
  • Uses effective service recovery skills for problem-solving.
  • Maintains responsibility for patient care plans through collaboration with PT/PTA team members.
  • Works collaboratively and communicates effectively with colleagues, respecting diverse opinions.
  • Uses resources efficiently and manages work schedules effectively.
  • Participates in continuous improvement initiatives and cost-reduction efforts.
  • Adapts treatments creatively based on team feedback and documentation.
  • Selects appropriate evaluations and treatment modalities, performs re-evaluations, and adjusts plans accordingly.
  • Assumes full management of patient progress from initial contact to discharge, maintaining primary responsibility for care plans.
  • Documents evaluations, progress notes, and discharge summaries in the EMR, reflecting patient responses and treatment outcomes.
  • Provides patient education tailored to individual understanding and motivation.
  • Participates in departmental committees, projects, and professional development activities.
  • Builds professional relationships with healthcare team members and participates in marketing efforts.
  • Fosters a respectful communication environment and accepts constructive feedback.
  • Completes non-patient care responsibilities timely, including scheduling, documentation, and administrative tasks.
  • Contributes to educational efforts and supports student clinical programs.
  • Maintains productivity and demonstrates flexibility to meet patient and department needs.
Qualifications

Required:

  • Bachelor’s Degree in Physical Therapy.
  • Licensed Physical Therapist in Illinois.
  • BLS certification from the American Heart Association within 90 days of hire.

Preferred:

  • Masters or Doctoral Degree in Physical Therapy.
