Join Our Team

We are seeking a part-time Outpatient Physical Therapist to join our rehabilitation team at the Lockport, IL clinic. The role involves planning, implementing, and evaluating patient care programs to enhance, maintain, or restore function, prevent disability, and help patients reach their maximum performance levels.

Essential Duties and Responsibilities

• Evaluate patients and gather relevant information for accurate diagnoses and treatment planning.
• Set functional and objective goals based on clinical examinations.
• Develop detailed treatment and care plans.
• Provide exceptional patient care and service.
• Maintain friendly, professional relations with clients and colleagues, fostering a positive work environment.
• Build relationships with patients and the community.
• Communicate regularly with referring physicians and other healthcare providers to ensure consistent care.
• Maintain patient records, clinical logs, and checklists to ensure compliance.
• Protect patient confidentiality in accordance with policies and HIPAA guidelines.
• Communicate effectively in professional settings.
• Perform additional duties as assigned.

Education and Experience

• Bachelor’s, Master’s, or Doctorate Degree from an accredited Physical Therapy Program.
• Current Illinois Physical Therapy license or willingness to obtain one.
• CPR certification.
