Job summary

The University of Vermont Health Network seeks a Pharmacist to ensure the appropriateness of drug therapy for inpatients and outpatients, maintaining all necessary records. You will support safe, effective medication use across care settings.

Licensed Vermont pharmacist with BS in Pharmacy and Doctorate preferred; ACPE-accredited program; residency or equivalent experience encouraged. Two years clinical hospital experience preferred.

Qualifications

• Bachelor of Science in Pharmacy from ACPE-accredited school.
• Doctor of Pharmacy degree preferred.
• Licensed Pharmacist in the State of Vermont.
• ASHP-accredited Pharmacy Residency or equivalent experience preferred.

Responsibilities

• Assure the appropriateness of drug therapy and meeting medication needs of patients.
• Maintain all necessary medication records.
• In the absence of a Manager, assume essential responsibilities and duties of the Manager.

JOB DESCRIPTION:

The Pharmacist assures the appropriateness of drug therapy as described in the position accountabilities. The pharmacist is responsible for ensuring that the medication needs of inpatients and outpatients are accurately met and for maintaining all necessary records. In the absence of a Manager, a Pharmacist may be required to assume the essential responsibilities and perform the duties of the Manager.

EDUCATION:

Bachelor of Science Degree from an ACPE accredited School of Pharmacy required. Doctor of Pharmacy Degree preferred. ASHP accredited Pharmacy Residency or equivalent experience preferred. Licensed Pharmacist in the State of Vermont

EXPERIENCE:

Two years clinical pharmacist experience in a health system preferred.
