Job summary

UVM Health - UVM Medical Center is seeking a Pharmacist who will assure the appropriateness of drug therapy and ensure medications for inpatients and outpatients are accurately met, while maintaining records. In the absence of a Manager, the Pharmacist may assume essential managerial duties.

The position requires a Bachelor of Science in Pharmacy, with a PharmD preferred, and Vermont licensure. Two years of clinical pharmacist experience in a health system is preferred.

Qualifications

• Bachelor of Science in Pharmacy required.
• Doctor of Pharmacy degree preferred.
• ASHP accredited Pharmacy Residency or equivalent experience preferred.
• Licensed Pharmacist in Vermont.

Responsibilities

• Assures the appropriateness of drug therapy as described in the position accountabilities.
• Ensures medication needs of inpatients and outpatients are accurately met.
• Maintains all necessary records.
• In the absence of a Manager, may assume essential responsibilities and duties of the Manager.

In the absence of a Manager, a Pharmacist may be required to assume the essential responsibilities and perform the duties of the Manager.

Education

Bachelor of Science Degree from an ACPE accredited School of Pharmacy required. Doctor of Pharmacy Degree preferred. ASHP accredited Pharmacy Residency or equivalent experience preferred. Licensed Pharmacist in the State of Vermont

Experience

Two years clinical pharmacist experience in a health system preferred.
