Job summary

Job Description

A full-time rotating shift inpatient clinical staff pharmacist will serve as an integral part of the multidisciplinary care team. Responsibilities include antimicrobial stewardship, medication safety, and optimization of pharmaceutical dosing. The pharmacist will assist with medication histories, monitor patient drug therapies, and provide drug information to nurses, physicians, and other medical staff. Core activities include order verification, pharmacokinetics, IV to PO conversions, therapeutic interchange, and other tasks as required.

Responsibilities

• Interpret and evaluate the validity and safety of medication orders and prescriptions.
• Integrate core clinical pharmacy knowledge, dispensing, and order review activities.
• Participate in multidisciplinary rounds, clinical initiatives, and educational/training activities.
• Serve as a preceptor to ASHP‑accredited pharmacy residents and pharmacy students.
• Assume leadership responsibilities when a leader is absent.
