Job summary

University of Vermont Health is seeking a pharmacist to assure appropriate drug therapy for inpatients and outpatients, maintaining essential records and ensuring medication needs are met.

The role requires VT licensure, a B.S. in Pharmacy (PharmD preferred), and ASHP residency or equivalent experience is encouraged. Two years of clinical pharmacist experience in a health system is preferred.

Qualifications

• Educational requirements: B.S. in Pharmacy required; PharmD preferred.
• Licensure: Licensed pharmacist in Vermont.
• Experience: 2 years clinical pharmacist experience in a health system preferred.
• Residency: ASHP accredited residency or equivalent experience preferred.

Responsibilities

• Ensure appropriateness of drug therapy and meet medication needs for inpatients and outpatients.
• Maintain all necessary pharmacy records and documentation.
• In the absence of a Manager, assume essential pharmacist responsibilities and duties of the Manager.

JOB DESCRIPTION:

The Pharmacist assures the appropriateness of drug therapy as described in the position accountabilities. The pharmacist is responsible for ensuring that the medication needs of inpatients and outpatients are accurately met and for maintaining all necessary records. In the absence of a Manager, a Pharmacist may be required to assume the essential responsibilities and perform the duties of the Manager.

EDUCATION:

Bachelor of Science Degree from an ACPE accredited School of Pharmacy required. Doctor of Pharmacy Degree preferred. ASHP accredited Pharmacy Residency or equivalent experience preferred. Licensed Pharmacist in the State of Vermont

EXPERIENCE:

Two years clinical pharmacist experience in a health system preferred.
