Job Description

A full‑time rotating shift inpatient clinical staff pharmacist role involving antimicrobial stewardship, medication safety, and optimized pharmaceutical dosing. Responsibilities include medication histories, monitoring drug therapies, providing drug information, interpreting and evaluating the validity and safety of orders and prescriptions, order verification, pharmacokinetics, IV to PO conversions, therapeutic interchange, and other required tasks. The pharmacist works closely with nurses, physicians, and other medical staff, and may assume leadership duties in the absence of a leader. The role participates in multidisciplinary rounds, provides direct patient care, educational and training activities, and may serve as a preceptor to ASHP‑accredited pharmacy residents and pharmacy students. The position is part of the 24\/7 inpatient pharmacy service and requires variable shifts, weekends, and holidays. Patient populations include oncology, surgery, medicine, cardiology, neurology, pediatrics, adult and pediatric critical care, emergency medicine, peri‑operative, psychology, same‑day infusions, renal, and rehabilitation units.

Benefits

Sign‑on bonus up to $5,000, paid over two years, and $6,000 relocation assistance may be available, subject to qualification.

Education

Bachelor of Science in Pharmacy from an ACPE accredited school (required). Doctor of Pharmacy (preferred). ASHP accredited Pharmacy Residency or equivalent experience (preferred). Licensed pharmacist in the state of Vermont.

Experience

Two years of clinical pharmacist experience in a health system preferred.
