Advocate Health in Wisconsin is seeking a Pharmacist for the Outpatient Pharmacy to deliver evidence-based pharmacotherapy, patient education, and safe medication use across the discharge and community services. You will participate in a robust meds-to-beds program and collaborate with the healthcare team to ensure high-quality care.

The role requires a pharmacy degree (ACPE) or FPGEC and a valid license with immunization certification.

Qualificações

• Licensed in the state(s) of practice or eligible for reciprocity
• Immunization and non-vaccine injectable certification required
• Credentialing and privileging required based on area of practice
• Bachelor’s or Doctorate degree in pharmacy from ACPE-accredited school or an FPGEC certificate

Responsabilidades

• Oversees pharmacological care, retrieving and evaluating biomedical literature and designing therapy regimens
• Provides patient education and counseling on medications, adherence, and administration
• Reviews medication orders and participates in formulary management
• Supports medication safety and quality improvement activities
• Prepares, dispenses, and oversees proper storage and handling of medications
