Qualifications

• Licensed pharmacist or eligible for reciprocity in practice state
• Immunization and non-vaccine injectable certification may be required
• Credentialing and privileging required based on area of practice
• Basic computer skills and pharmacy software familiarity may be required

Responsibilities

• Oversees pharmaceutical care; retrieves and evaluates biomedical literature; designs medication therapy regimens
• Provides patient education and counseling on therapy, adverse effects, compliance, use and administration
• Designs, monitors and evaluates patient-specific regimens based on literature; implements and documents accordingly
• Reviews and interprets medication orders and prescriptions accurately
• Participates in management of medical emergencies when applicable
• Supports medication safety and quality improvement activities; reports events
• Performs quality assurance and performance improvement of the medication use system
• Collaborates on formulary adherence and medication-use processes; assists with non-formulary/compassionate/investigational meds
• Complies with organization policies and regulations
• Prepares, dispenses, and oversees storage and handling per best practices and regulations
• Serves as preceptor for students and residents; mentors staff
• Directs and checks accuracy of pharmacy support team; ensures safe workflows
• Provides age-specific care per department standards and policies
