Responsibilities

• Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens
• Counsels patients and caregivers on medication therapy, proper use, and handling
• Designs and monitors pharmacotherapeutic regimens based on literature
• Reviews and interprets medication orders and prescriptions
• Participates in management of medical emergencies when applicable
• Supports medication safety and quality improvement activities
• Collaborates on formulary adherence and procurement of non-formulary items
• Ensures compliance with policies, procedures, and regulations
• Prepares, dispenses, and ensures proper storage/handling of medications
• Serves as preceptor for pharmacy students and residents
• Mentors and trains pharmacy teammates and learners
• Directs accuracy checks by pharmacy support staff
• Assesses patient age-specific needs and care requirements
