Advocate Health is seeking a Physical Therapist for the High Point Medical Center Outpatient Rehabilitation: Physical Therapy Elm. The role is full-time (40 hours/week) with Mon-Fri scheduling and occasional Saturday coverage.

You will assess and treat patients, document care, and coordinate with the care team to ensure high-quality outcomes. Must hold a DPT/MS/PT from an accredited program, maintain active state licensure, and possess CPR/BLS certification.

Qualifications

• Must hold a degree in physical therapy (DPT/MS/PT) from an accredited program.
• Current licensure in the state of practice is required.
• CPR (BLS) certification is required.

• Provide patient care assessment and treatment within standard practice.
• Maintain accurate documentation including evaluations, progress notes, and discharge summaries.
