Job summary

Advocate Health is seeking a Full-time Physical Therapist for the High Point Medical Center Outpatient Rehabilitation: Physical Therapy Elm. This role requires a PT degree (B.S./M.S./D.P.T) and current state licensure, with CPR (BLS) certification. Mon-Fri with occasional Saturdays.

Qualifications

• Must have B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of practice.
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Adheres to hospital standards and policies; promotes teamwork and quality patient care.
• Provides patient care assessment and treatment with functional, goal-oriented plans.
• Maintains accurate documentation of evaluations, progress notes and discharge summaries.
• Meets department financial and productivity standards and adjusts schedules as needed.
• Participates in process improvements to enhance quality and efficiency of services.
• Promotes professional development and education of staff and representatives on committees.
• Communicates effectively with patients, physicians, staff, and external customers.
• Delivers age-appropriate patient care and adheres to age/developmental guidelines.
