Job summary

Advocate Health in Charlotte, NC seeks a Physical Therapist for outpatient rehab. You will evaluate patients, develop and implement plan of care, and educate patients and families. Strong ortho/sports PT experience preferred, with a focus on evidence-based practice and multidisciplinary communication.

Qualifications

• Bachelor’s degree in Physical Therapy and state license are required.
• BLS certification required.
• Orthopedic/sports PT experience preferred.

Responsibilities

• Evaluates patients, selects assessment techniques, and plans care including modalities and education.
• Gathers data from medical records and care team to inform treatment.
• Establishes functional goals and provides evidence-based interventions with ongoing assessment.
• Documents evaluations, progress notes, discharge notes, and charges as needed.
• Communicates progress to physicians and coordinates treatment and discharge planning.
