Advocate Health in Charlotte, NC, is seeking a full-time Physical Therapist for Randolph - Outpatient Rehabilitation. You will evaluate patients, design and implement evidence-based treatment plans, and coordinate care with physicians and staff to support recovery and discharge planning.

Bachelor's degree in Physical Therapy and an active state license are required, with BLS certification. Strong orthopedic/sports PT experience is preferred; supervision of PTAs and Rehab Techs may be involved,

Qualifications

• Bachelor's degree in Physical Therapy and state license
• Current BLS per policy
• Strong ortho/sports PT experience preferred

Responsabilités

• Evaluates patients, selects and administers assessment techniques, and establishes patient care plans including modalities, exercises, and education.
• Gathers and interprets data from medical records, physician reports, patient and family with proper documentation.
• Sets functional goals with the patient and provides evidence-based interventions with ongoing plan-of-care adjustments.
• Documents evaluations, tests, progress notes, discharge notes, and charges as needed.
• Communicates evaluation results and progress to physician, referral sources, and other staff; coordinates schedules and discharge planning.
• Supervises PTAs and Rehab Tech/Aide as required for licensure.
