• Evaluates patients and develops care plans based on diagnosis, age, and discharge needs.
• Gathers and interprets data from the medical record, physician reports, patient, and/or family.
• Establishes functional goals and provides evidence-based interventions with ongoing reassessment.
• Documents evaluations, tests, progress notes, attendance records, patient/family education, outcomes, discharge notes, and charges.
• Communicates patient’s evaluation and progress to physicians and care teams, coordinates schedule and discharge planning.
• Provides supervision of PTAs and Rehab Tech/Aide as required for licensure.
