Job summary

Advocate Health is seeking a Physical Therapist to join a busy hospital team in North Carolina. The role requires a B.S., M.S., or D.P.T from an accredited PT school, current licensure in the state, and CPR/BLS Level C certification.

The successful candidate will perform patient assessments, develop and implement treatment plans, and document care with attention to standards and outcomes.

Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of applicable state.
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Promotes a cooperative work environment through communication, relationships and team building; adheres to hospital policies and procedures.
• Provides patient care assessment and treatment with standard of practice and realistic goals; documents progress and adjusts care as needed.
• Maintains accurate documentation of evaluations, progress notes, discharge notes and education; ensures proper ICD-10 coding and charting.
