Job summary

Advocate Health in Charlotte’s Carolinas Rehabilitation outpatient setting seeks a licensed Physical Therapist to evaluate patients and develop holistic care plans. You will select assessment techniques, administer modalities, and guide exercise programs with patient education tailored to diagnoses and discharge needs.

You will document findings, set functional goals, and adjust plans as needed while coordinating with physicians, staff, and families.

Qualifications

• Bachelor's degree in Physical Therapy program required.
• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Evaluates patients and develops a plan including modalities, exercise programs, and education.
• Gathers data from the medical record, physician reports, patient, and/or family and documents.
• Establishes functional goals with the patient and provides evidenced-based interventions with ongoing reassessment.
• Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges.
• Communicates evaluation and progress to physician, referral sources, and other clinical staff; coordinates patient’s schedule and discharge planning.
• Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
