Job summary

Qualifications

• Ability to perform initial examinations and develop diagnosis, prognosis, and plan of care.
• Ability to document and communicate plan of care and progress to care team.
• Discharge planning and coordination with referring providers.

Responsibilities

• Perform initial examination to establish impairment, prognosis, and plan of care.
• Provide, or supervise, therapy interventions aligned with exam results.
• Coordinate discharge planning and follow-up with patients and teams.
• Document all aspects of patient management and status changes.
• Supervise rehab aides and support staff; manage caseload and scheduling.
