Job summary

Adventist HealthCare Home Health in Silver Spring seeks a Physical Therapist (PT) to extend care across rehab settings. Eligible candidates may receive up to a $12,000 sign-on bonus (current AHC employees not eligible).

The clinician will perform examinations, develop plans of care, lead therapy interventions, and coordinate with the care team. Full-time, 40 hours/week, 8–5, with monthly weekend requirements.

Qualifications

• Must have a PT degree (Bachelors/Masters/Doctorate) in Physical Therapy.
• 1-year site-specific experience preferred.
• Active Maryland License required.
• Valid driver’s license with agency insurance requirements.
• Active American Heart Association Basic Life Support (BLS) certification required.

Responsibilities

• Perform initial examination and evaluation to establish impairment/diagnosis, prognosis and plan of care for rehabilitation needs.
• Provide, or supervise, therapy interventions aligned with the plan of care and involve patient in planning and assessment.
• Plan for discharge considering goals and outcomes; ensure appropriate follow-up or referral.
• Communicate, coordinate and document all aspects of patient management with the care team; supervise rehab aides/assistants.
• Manage schedule to accommodate census and caseload; inform management of issues.
• Attend team meetings and communicate patient status; pursue professional development.

Education

Bachelor/Master/Doctorate in Physical Therapy

1-year site specific experience

Active Maryland License

Valid driver’s license

American Heart Association Basic Life Support (BLS) certification
