Job summary

Advocate Health seeks an experienced Outpatient Infusion/Oncology Clinical Pharmacist to join our oncology pharmacy team at Advocate Christ Medical Center. This onsite role focuses on high-volume infusion care, optimizing medication management for cancer and specialty treatments while collaborating closely with the healthcare team.

The ideal candidate has 3–5 years of outpatient oncology experience or related infusion center workflow; BCOP is preferred but not required.

Qualifications

• Pharmacist degree from ACPE-accredited school or NABP FPGEC certificate.
• Licensed in the state(s) of practice or eligible for reciprocity.
• Board Certification in Oncology Pharmacy (BCOP) preferred, but not required.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Provides patient and caregiver counseling on medications, effects, and administration.
• Develops and monitors pharmacotherapeutic regimens using biomedical literature.
• Interprets medication orders and prescriptions accurately.
• Supports medication safety and quality improvement activities.
• Manages formulary adherence and procurement of non-formulary or investigational medications.
