Job summary

Licensure in the state, ACPE-accredited PharmD, and BCOP preferred but not required.

Qualifications

• Pharmacy degree from ACPE-accredited school or NABP FPGEC certification.
• Illinois licensure or eligibility for reciprocity.
• Board Certification in Oncology Pharmacy (BCOP) preferred but not required.

Responsibilities

• Oversee pharmaceutical care in high-volume infusion setting.
• Counsel patients and caregivers on therapy, adverse effects, and adherence.
• Design and monitor patient-specific pharmacotherapeutic regimens.
• Review medication orders for safety and effectiveness.
• Collaborate with healthcare team on formulary and process improvements.
• Serve as preceptor for students and training staff.
