Hartford HealthCare in Norwich, CT is seeking an Occupational Therapist to evaluate patients, develop and implement individualized plans, and promote independence across daily living, leisure, and work activities.

The role includes supervising OT students and rehab staff, delivering evidence-based therapy, and thorough documentation of progress and outcomes, while coordinating with the health-care team to support smooth patient transitions.

Qualifications

• ACOTE-accredited OT degree required.
• OT license to practice in CT, CPR certification, NBCOT certification.

Responsabilités

• Evaluates patient level of function and develops individualized OT treatment goals.
• Implements therapy to maximize independence in daily living, leisure and work.
• Supervises OT students, Rehab Aides and Certified OT Assistants.
• Documents evaluation, care plan, progress and discharge summaries.
• Coordinates care with Rehab Therapists, Nurses, Social Workers and other resources; supports discharge planning.
