Job summary

Hartford HealthCare is seeking an Occupational Therapist in Southington, Connecticut. You will evaluate and implement therapy for patients to promote functional independence in daily living and work.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current Occupational Therapist License is required.
• Preferred experience in mental health treatment and group facilitation.

Responsibilities

• Evaluate patient’s level of function upon Physician referral.
• Establish measurable treatment goals with patients and families.
• Implement and deliver patient care using therapeutic interventions.
• Provide education about treatment and equipment use.
• Document patient evaluations, care plans, and progress.
• Participate in discharge and transition planning.
• Coordinate care with other healthcare providers.
