Responsibilities

• Perform comprehensive evaluations of a patient’s level of function.
• Create measurable goals and modify care plans as needed.
• Deliver OT techniques across inpatient, outpatient, home care, SNF, or assisted living settings.
• Coordinate with rehab team to ensure seamless transitions of care.
• Complete documentation including evaluation, plan, progress notes, and discharge summaries.
