Job summary

Oak Street Health in Lexington, KY, seeks a Nurse Practitioner to provide value-based primary care to adults on Medicare in an outpatient setting. You will work with a collaborative care team and focus on longitudinal care, patient relationships, and population health.

The role reports to the Center Medical Director and offers a full-time schedule with a comprehensive benefits package.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
• Active, non-probationary state Nurse Practitioner license.
• Active DEA license; US Work Authorization.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers, facilities, and agencies.
• Lead population health initiatives with the care team.
• Assist with phone triage and outreach.
• Educate patients on their health conditions, care plans, and treatments.
• Participate in Oak Street Health promotions.
• Conduct home visits as needed.
• Other duties, as assigned.
