Job summary

Oak Street Health in Lexington, KY is seeking a Nurse Practitioner to provide value-based primary care for adults on Medicare. You will see patients independently within a collaborative care team and contribute to care plans and population health initiatives.

Responsibilities include office visits, annual wellness visits, health risk assessments, and comprehensive care coordination. The schedule is full-time, 40 hours weekly, with a strong emphasis on teamwork and patient relationships.

Qualifications

• Master of Science in Nursing required or equivalent.
• National NP certification in one of the specified specialties.
• Active, non-probationary NP license in the US and DEA license.
• US work authorization required.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs with thorough documentation.
• Coordinate care with physicians, specialists, labs and facilities.
• Lead population health initiatives in coordination with the care team.
• Assist with phone triage and outreach.
• Educate patients on conditions, care plans and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Perform other duties as assigned.
