Job summary

Oak Street Health is seeking a compassionate Nurse Practitioner to deliver value-based primary care to Medicare patients in our Lafayette outpatient clinics. You will work within a collaborative care team to keep patients healthy, out of the hospital, and engaged in their care plans.

The schedule is full-time, Monday through Friday, 8:00 AM to 5:00 PM, with ongoing opportunities to coordinate with physicians, nurses, and specialists across our network.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
