Qualifications

Responsibilities

• Perform Evaluation/Re-evaluation with patient and family.
• Gather data and conduct examination to determine treatment.
• Develop and adjust an individualized plan of care with patient and family.
• Coordinate care with interdisciplinary team and establish discharge plan.
