Job summary

Fairview Health Services seeks an occupational therapist specializing in lymphedema therapy to join our rehab teams at the Southdale Place Clinic in Edina and the Rehab Clinic in Eden Prairie. The role emphasizes comprehensive evaluation, treatment planning, and documented care delivery within scope of practice.

Responsibilities include patient/family collaboration, data gathering, prognosis, goal setting, and coordination with the interdisciplinary team.

Qualifications

• Minnesota OT license required or temporary license.
• BLS certification by American Heart Association or Red Cross.
• Lymphedema certification preferred or in process within one year of hire.

Responsibilities

• Perform evaluation and re-evaluation of patients.
• Develop and implement an individualized plan of care.
• Coordinate care with patients, families, and interdisciplinary team.
• Document and bill for services in a timely manner.
• Meet productivity and quality improvement standards.
