Responsibilities

• Evaluate and re-evaluate patients to determine treatment diagnoses.
• Develop and implement individualized plans of care with patient/family collaboration.
• Monitor patient progress and adjust treatment plans as needed.
• Coordinate care with interdisciplinary team and communicate discharge plans.
• Provide patient education to support learning needs and self-management.
• Ensure safety and compliance with scope of practice.
