Job summary

Presbyterian Healthcare Services in New Mexico seeks an experienced RN Case Manager for hospice services in outpatient and home settings. You will coordinate care through a multidisciplinary team, ensuring timely assessments and effective care plans to achieve quality patient outcomes.

The role requires an Associate Degree in Nursing, NM RN licensure, BLS, and a minimum of one year in direct patient care. Precepting duties and strong documentation are included.

Qualifications

• Associate Degree in Nursing
• Registered Nurse-NM
• Minimum one year in direct patient care setting or home health care.
• License to practice as Registered Nurse in the state of New Mexico
• Current drivers license and auto insurance.
• BLS required

Responsibilities

• Provides skilled nursing care and case management of hospice patients in home/community settings.
• Coordinates patient care through collaboration with patient/caregiver, multidisciplinary team, and physician.
• Documents care plans timely and in accordance with regulations and policies.
• Ensures plan of care is followed and documentation is compliant with regulatory requirements.
• Precepts newly hired nurses and serves as a resource and role model.
• Maintains communication with the care team and updates schedules as needed.
